LRS INSTITUTE OF TUBERCULOSIS & ALLIED DISEASES
6.40.1. LRS Institute of TB & Allied Diseases, New Delhi is an autonomous Institute under Ministry of Health and Family Welfare, Govt. of India. In addition to providing patient care for T.B. & other respiratory diseases, the Institute is actively involved in teaching, training and research in the field of T.B. & allied diseases. The Institute assists the Government of India in developing strategies for T.B. control and in implementation of the same. The Institute has a daily OPD with an average attendance of 450 patients and Indoor Wards having 500 beds. The Institute has a well equipped Respiratory Intensive Care Unit & Thoracic Surgery Deptt. with state of art of equipment. Besides this, the Institute has various support departments like Microbiology, Pathology, Radiology, Paediatric tuberculosis and pulmonology, clinical respiratory physiology (PFT Lab.), Anaesthesia, Epidemiology, Biostatistics, Health Education and Computer facility etc.
6.40.2. O.P.D. Clinics: The patients attending the LRS OPD come from Delhi as well as from the adjoining states like Haryana, Rajasthan, U.P., Bihar etc. The Non-Area and Outside Delhi patients are diagnosed and advised to attend their area T.B. clinics for further management.
6.40.2.(i) In the specified area of the Institute covering a population of over I million, 9 DOTS centres under RNTCP are functioning in different localities. This facilitates the TB patients to take regular treatment nearer to their houses.
6.40.2.(ii) The area patients not covered by the DOTS Centres, take domiciliary treatment from the OPD. These are considered as LRS-OPD Area cases.
6.40.2.(iii) Patients Data (April 98 Oct. 98): During the period from Ist April, 98 to 31st Oct. 98, a total of 18516 suspected and referred patients visited the OPD. Out of these, 3615 patients visited under DOTS centres, and 14901 patients visited in LRSOPD.
6.40.2.(iv) LRS - OPD (Area + NonArea + Outsider): Among 14901 suspects, a total of 3229 (87%) pulmonary tuberculosis and 478 (13 %) Extra Pulmonary tuberculosis cases were diagnosed. Out of 3229 PT cases, 1764(54.6%) were sputum positive and 1465 (45.4%) were sputum negative.
6.40.2.(v) DOTS Centres (RNTCP - Data): Among the 3615 susp- ects, who visited the DOTS Centres, a total of 537 cases of pulmonary tuberculosis and 114 cases of Extra Pulmonary tuberculosis were diagnosed. Among 537 PT cases 3 86 (72%) were sputum positive and 151 (28%) were sputum negative.
6.40.2.(vi) Total TB cases: A total of 3766 cases of Pulmonary tuberculosis and 592 cases of Extra Pulmonary tuberculosis were diagnosed comprising a total of 4358 TB cases. Out of 3766 PT cases, 2150 (57.1%) were sputum positive and 1616 (42.9%) were sputum negative.
6.40.3. Indoor facility (April to Oct' 98): A total of 830 patients were admitted in the hospital during April, 98 to Oct. 98 for indoor treatment as they either had severe extensive disease or complications or required surgical interventions.
6.40.3.(i) A total of 1,201 surgical procedures were carried out, out of these 199 were major procedures which includes Lobectomy, Pneumonectomy, Decortication, open Lung Biopsy, Thoracoplasty, Bronchos- copy, intercostal Intubation etc.
6.40.3.(ii) Respiratory Intensive Care Unit: Dhanvantri Respiratory Intensive Care Unit is catering to the patients with respiratory diseases requiring intensive care. It is a 14 bedded ICU equipped with the State of Art equipment such as a ABG cum electrolyte analyser, conventional ventila-tors, Defibrillator, Nebuliser, Bronchoscope and portable X-ray machine. A total of 173 patients were admitted in ICU during the period April to Oct'98. More than 942 ABGs were carried out.
6.40.4. Mobile TB Detection Van: The Institute has a mobile TB detection VAN equipped with X-ray machine, sputum microscopy and drug distribution facility. This is being used for the field work in far flung slums and villages.
6.40.5. Teaching and Training: The Institute has been providing teaching and training to the post graduate students (DTCD, M.D.) of V.P. Chest Institute, AIIMS, Safdarjung Hospital, New Delhi.
6.40.5.(i) Training is one of the main objectives of the LRS Institute and over the years, the Institute has been actively involved in training on the Revised National Tuberculosis Control Programme. In addition, the Institute has also been organising workshops, seminars, CME programmes and symposiums on RNTCP. Till date nearly 900 personnel including key trainees from other States have been trained at Institute. During the first six months of 1998-99, 45 doctors, 21 Senior Treatment Supervisors, 21 Senior TB Lab. Supervisors and 71 Lab Technicians were trained in the Institute on the Revised National TB Control Programme.
6.40.6. Major Research projects: The Institute has conducted an operational research study funded by DFID (Department for International Development "To assess the needs and Perspectives of TB patients and providers of tuberculosis care in Nehru Nagar and Moti Nagar Chest Clinic areas in Delhi". This was done in collaboration with London School of Hygiene 'and Tropical Medicine (LSHTM), London, U.K. and Foundations for Research in community Health (FRCH) Bombay. The final report has been submitted to DFID
6.40.7. C. T. Scan: A whole body spiral C.T. Scan was commissioned in the Institute on 18th August, 1998 with the inauguration by Hon'ble Union Minister of State for Health & Family Welfare Sh. Dalit Ezhilmalai. This costly diagnostic modality has now come within the reach of the poor T.B. patients.
6.40.8. Future plans: The Institute has identified the following key areas as priority under the Ninth Five Year Plan.
6.40.8.(i) Training: The training division of the Institute is impto a 'Thoracic Surgical Centre'.
6.40.8.(ii) Teaching: Teaching is one of the the main objectives of the Institute and the teaching faculty is being strengthend in orfder to start the teaching programmes like Diplomate of National Board and Regular teaching of under- graduate post-graduate studennts in the field of T,B, & allied diseases is alo being planned.
6.40.8.(iii) Thoracic Surgical Centre: The Thoracic Surgical department of the Institute is one of its kind in Northern India and will be upgraded suitabley in terms of manpower and equipments into a "Thoracic Surgical Centre".
6.40.8.(iv) The Administration and Accounts sections which are imperative to support different departments of the Institute will be suitably strengthened.
6.40.9. Budget: The Budget estimate under the plan and non-plan heads for the year 1998-99 are Rs. 6.50 crore and Rs. 2.97 crore respectively.
